    Petang hari Isnin yang lalu, tetiba aku rasa sangat lemah, seolah-olah mahu pitam. Mungkinkah kandungan gula dalam darah tinggi atau aku mengalami dehidrasi kerana kekurangan air. Apapun, petang itu, aku tahu aku tak dapat drive balik, so aku call suami untuk mengambilku dari tempat kerja. Kami terus ke Hospital UMRA untuk mendapat pemeriksaan kesihatan. Dadaku memang berdebar-debar, seolah-olah ada sesuatu yang tidak kena. Semasa giliranku dipanggil, aku pun menceritakan perihal yang berlaku kepada doktor. Doktor menasihatkan aku menjaga pemakanan, melakukan senaman ringan seperti berjalan untuk mengatasi masalah keletihan yang aku alami.


    Doktor bertanyakan sama ada aku ingin melakukan detail scan memandangkan kandunganku hampir mencecah 5 bulan dan kebetulan, ada pakar scan bertugas. Aku mengiyakan sahaja sebab memang aku agak risau.


    Pada mulanya, pakar scan mengatakan bahawa air ketuban atau amniotic fluid aku tiada, mungkin bocor. Kemudian doktor mula mengambil ukuran-ukuran organ dan tulang. Pada masa doktor mengesahkan ada sesuatu yang tak kena pada kidney kandunganku, terasa seperti mahu tercabut jantung dari badan. Doktor mengatakan yang kandunganku menghidapi polycystic kidney, di mana kidney itu bersaiz lebih besar dari normal dan terdapat ketulan-ketulan yang dikenali sebagai cyst di kedua-dua buah pinggang kandunganku. Ya Allah, kedua buah pinggang kandunganku gagal berfungsi, dan itu lah yang menjadi punca mengapa tiada air ketuban. Aku kemudian diberi surat rujukan untuk ke Pusat Perubatan Universiti Malaya (PPUM) untuk berjumpa sonographer specialist di sana untuk mendapat pandangan kedua.


    Esoknya, kami ke PPUM tapi gagal menemui specialist kerana tiada specialist yang available. Kami diberi temujanji untuk kembali keesokannya. Hari Rabu, 30 Mac 2011, seawal jam 7.00pagi kami sudah sampai ke PPUM tapi kaunter nombor giliran hanya dibuka jam 7.30pagi. Sememangnya bermacam-macam bermain dikepalaku. Aku memang dapat mengagak apa yang bakal berlaku dan mahu bersedia dari segi mental untuk mendengar sebarang kemungkinan yang berlaku.


    Pukul 8pagi, nomborku dipanggil. Selepas membuat pembayaran, kami terus diarah ke bahagian Ultrasound. Tapi kami terpaksa menunggu hingga pukul 9pagi memandangkan doktor hanya datang selepas waktu itu.


    Ketika giliranku dipanggil untuk melakukan ultrasound, segalanya bagai mimpi. Satu demi satu ayat-ayat yang pernah kudengar dari mulut pakar sebelum ini diulang. Doktor mengatakan ada tiga sebab mengapa kidney kandunganku boleh rosak.


    Satu kemungkinan sejarah family aku atau suami pernah menghidapi masalah kronik buah pinggang yang perlu melalui dialysis... kemungkinan aku dan suami mungkin adalah pembawa, ini dikenali sebagai polycystic kidney, dimana imejnya akan nampak seperti satu objek yang solid.


    Kedua, mungkin aku adalah pembawa, dan masalah itu tidak dihidapi aku tapi diturunkan pada kandunganku, tapi kebiasaannya kalau kes sebegini, baby hanya akan menunjukkan simptom bila dewasa. Ia dipanggil sebagai Adult polycystic kidney.


    Ketiga, aku dan suami bukan pembawa, tapi ia terjadi juga secara random. Biasanya hanya akan effect salah satu kidney tapi ada juga kes yang effect kedua-dua belah kidney. Ini dekenali sebagai multi-cyst.


    Melihat dari segi pattern dan sejarah keluarga kami berdua, doktor mengatakan kemungkinan kandunganku menghidapi simptom yang ketiga, iaitu multicyst kidney.


    Kerosakan kedua-dua belah kidney menyebabkan baby tidak mampu kencing, dan menghasilkan air ketuban. Ketiadaan air ketuban akan menyebabkan paru-paru baby gagal membentuk (kecil dan keras), tidak mampu untuk mengembang. 10% baby akan survive sekiranya dimasukkan air ketuban tambahan dan jika tiada jangkitan dan jika paru-paru baby dapat berkembang. Tetapi disebabkan kidney baby keduanya tidak berfungsi, peluang untuk baby survive tiada. Sekiranya salah satu kidney masih berfungsi, kandunganku mungkin mempunyai peluang untuk hidup.


    Doktor memberikan kami pilihan, jika diteruskan kandungan, bayi akan hidup selama dia berada di dalam rahim, dan akan meninggal bila dilahirkan. Disebabkan tiada air ketuban, pembedahan akan dilakukan pada bahagian atas rahim(kerana rahim kecil, dan kandungan juga mungkin kecil). Risiko rahim akan pecah dan meninggikan risiko pada kandungan akan datang.


    Pilihan yang paling sukar ialah terminate the pregnancy. Untuk menjaga rahim pada kandungan seterusnya. Bersalin secara normal (induce dengan ubat).


    Berat mata memandang, berat bahu memikul. Selama ini tidak pernah terbayang bagaimana perasaan ibu-ibu yang mengalami nasib sebegini. Kami harap dugaan ini bakal membuatkan kami lebih mensyukuri kurniaan-Nya dan dapat bersabar menghadapi ujian ini. Walaupun baby hanya dipinjamkan untuk berada beberapa bulan cuma di rahimku, kami harus terus bersyukur dan tabah. Hanya Allah yang Maha Mengetahui dan semua alam maya ini adalah milik-Nya.

    Yay.. today is my birthday. Thank you Allah for giving me a chance to celebrate it one more time, with my beloved husband, family and friends.

    Actually we have no plan for the day. Mr Hubby still haven't get his salary. Fortunately I already got mine a day before.. hehe..

    So this morning, after having breakfast, I bought the movie ticket to see the Hikayat Merong Mahawangsa aka the Malay Chronicles. I was a bit skeptical at first, but the trailer do seem very interesting. I was afraid that I would be dissapointed like watching Cicak Man.. why.. because despite having the movie in CGI, the script was horibble... major dissapoinment... but after watching Merong Mahawangsa....



    I was actually feeling a glow of pride on how our cinematic industry has grown. I love the story line (altho at some part a bit off), the script also has seen lot of improvement, the CGI great, the casting excellent, especially for Merong Mahawangsa character and Kamawas. And there's bit of humor... cleverly inserted between the line. I want to give my compliment to KRU production for their bold step in raising the bar of our local movie industry. I actually enjoyed the movie much..much better than Clash of the titans which I think sucks a lot...

    After having good time at the movie (early bird ticket, so only few people in the cinema)... we have lunch @ Chicken Rice Shop where I have a nice Steam Chicken Rice (was a bit angry at the waitress because she took like forever to bring me extra chillies.. I normally don't get so work up on things like that, maybe because of hormon). Then we went to Jusco to redeem my Birthday Gift Voucher where I also get RM10 voucher from the Jusco card point (spent in on delicious Korean Strawberry - RM10.90).. and went home as I was feeling very tired already...

    So that's all I do today on my birthday. Yesterday also we have a nice birthday celebration in the office where Micheal treat us with Pizza Hut and New York Cheese Cake...

    I have a very happy birthday this year and I am very happy to get all those warm birthday wishes!! Thank you!!

    Dua tiga menjak nie mood tak bess... stress je memanjang.. xtao punca apa?

    maybe sebab rasa xcukop rehat, ada ja aktiviti time weeken... weekday plak keja banyak... stress... boss nak projek jalan... requirement tak complete... production request itu ini nak kena layan..

    Baru2 nie boss bagi cadangan keje 6 hari seminggu?? nak cover schedule... alamak..camner...

    buat BSP test untuk check gula, ada ja satu bacaan akan mencelah bagi kantoi.. tak masa awal pagi sebelum breakfast, kena plak after lunch.. akibatnyer.. tetiap minggu kena jumpe doctor.. government clinic tahu2 jalah.. ramainya orang.. lamanya tunggu... stress betul..

    Cuti dah tinggal sket... asek amik time off untuk g klinik... keje plak cam jauh sangat dah delay...

    aduhai... harap2 baby terus bertahan... moga2 stress nie xbagi ape2 effect negative kat baby...

    nak sangat lepak bercuti tanpa perlu pikir apa2.. bila tu yek.. tak tahulah... :(

    Yesterday was another day for my clinic visitation to see doctor. Because of having gestational diabetis melitus (GDM), I was schedule to see the doctor once a week until I can get my blood sugar normal, then the visitation would ceased to one per two weeks time.

    well... the waiting time was horible because there was a lot of people at the clinic... by the time I reached there around 7.45am, it was already pack of people. It was quite annoying but i undersatnd the situation. It was something that I need to tolerate, because of the free treatment. Our finacial still not too good due to unexpected expenses on car repairment. And despite the queues which sometimes look like forever, the service is good. I mean each visitation, I get my weight, blood pressure, test blood and urine and have ultrasound... all for free... :)

    Last visitation also I received the injection (its hurt laa... long time didnot get injected on the shoulder arm) for "kancing gigi" due to starting to feel baby movement or what they call as "quickening".

    Okay.. actually I wanna story about one incident that happen while I was waiting at the lab to get my blood and urine tested. Behind me seat an aunty which I assume age aroung early to middle of 40s. Can't help but eavesdropping as she spokes very loud. There's a girl turn to go and "give" her urine sample. The lab's lavatory is occupied, so she take the urine cup to other toilet. That's when I hear this aunty saying

    "Haa.. tengok.. bodohnye budak tu, mana boleh pergi tandas lain, kena guna tandas makmal.. nanti macam mana nak bagi air kencing tu"
    "Kalau air kencing tumpah macam mana... bodoh betul"
    and she keep on nagging on how stupid the girl was... the way she talk really is annoying, as if she's the smartest person there and other people stupid...

    when the girl came back with the urine in the cup, she wanna give it at the counter, and the attendant told her to put it at the designated place. No harm done right... but the aunty keep on complaining saying

    "kan dah cakap. Kita yang duduk kat sini pun tahu.. apa lah bodoh sangat itu pun tak tahu"

    This kind of person also exist. I don't think that the girl act equal to stupidity, she just don't know and was quite resourceful as she able to seek other alternative than waiting on an occupied lavatory. If we don't know.. we ask. By asking, it doesn't mean we are stupid, it just mean that we need assistance to things we are not familiar...

    The aunty still complaining about god-knows-who and how stupid the person is when my name was call for me to proceed to test sample. "Thank God" :)

    What doctor told me was everything is find except for my sugar level and the placenta is lower that it should be. She said it might not be anything to worry about as the placenta will go up later and she will check the progress (scan) by monthly to ensure everything fine. :)

    Late entries... hehe.. super duper bz week. With the clinic visitation, work, work, work and wedding.... haaaaaaaaa... super duper tired as well..

    Talk bout the breastfeeding talk, it is one of the must hear talk during pregnancy. So I was schedule to attend it on that Friday evening, while Mr Hubby already in kajang for my SIL wedding.

    So what do I get from the talk?? Quite a lot considering I am a first time mom and has no experience at all. I just wrote down what I remembered:-

    - Breastfeeding is very important for you child as it has all the nutrient that is needed for baby growth (such as Colostrum(for imunisation and only available during 1-2 days after labour), DHA (for eyes and brain development))
    - maintain a good relationship between mother and child
    - cost saving :p
    - higher IQs
    - Good for mother as it will take care of your "rahim" and can be used as family planning for the first 6 month (*t&c applied :p)
    - milk are assure not contaminated (direct feeding)
    - save time (no need to wake up and prepared milk)
    - prevent swollen breast
    - the more you breastfeed, the more milk you will produce
    - breastfeed exclusively for the first 6 month (no need additional food, supplement, water, breast milk is sufficient enuff)
    - Feed by demand not by time
    - additional food can be given when baby is more than 6 month but to continue breastfeed until 2 years is the best way

    The talk also teaches us how to breastfeed infant correctly and how to use the breast pump.

    Bad news all over again. Yesterday did the BSP test and the result not too good. So my session with the doctor today actually addresses the concern and important of how I should control my sugar level to reduce the risk to my precious one.

    Worry? Yes I am. The stress also not really helping. Stress never is good for pregnancy and having Mr Hubby sulking to me last night just make me more stressful. I really need to know how little junior is doing but I am only schedule for another scan this Wednesday, after undergoing the BSP test again (this time I ask doctor to let me do it at home using the home glucose meter) on Tuesday. Doctor warn me if I have another bad reading, I would need to be admitted to the hospital. I really don't want that.

    To ease my mind, I think I am gonna go to UMRA again today for private scan. Can't wait until Wednesday to know. I know that is not good to undergo scan procedure frequently, but I just need to know.

    Gestational Diabetes
    What is gestational diabetes?

    Gestational diabetes is the diabetes mellitus that develop for the first time during pregnancy. It disappeared 6 weeks after delivery.

    How does it occur?
    During pregnancy, the placental secretes 3 anti-insulin hormones ® estrogen, hPL(human placental lactogen) & cortisol. These hormones against the function of insulin (The function of insulin is to reduce the blood sugar level). To counter this effect, the pancreas produces and secretes more insulin. Thus, the level of insulin is raised by the action of the placental hormones, placing a burden on the insulin-secreting cells (pancreatic islets). The pancreas may be unable to meet these demands in women genetically predisposed to develop both types of diabetes.

    There are 2 scenarios of diabetes in pregnancy:

    · Established diabetes

    Women who are already diabetic at the start of pregnancy (most cases)

    · Gestational diabetes

    Development of diabetes for the first time during pregnancy.

    Gestational diabetes may or may not disappear following delivery. Repeated pregnancy may increase the likelihood of developing permanent diabetes, particularly in obese women. Studies have shown that some 80% of women with gestational diabetes ultimately develop permanent clinical diabetes requiring treatment.


    How to confirm gestational diabetes?
    It is diagnosed by a test, called Modified Oral Glucose Tolerance Test (MOGTT), which show raised blood sugar level.

    Procedure for MOGTT:

    • MOGTT should be done in the morning after at least 3 days of normal diet and usual physical activity

    • The test should be proceed by an overnight fast of 10-16 hours during which plain water may be allowed

    • Subjects should be at rest for the duration of test. Smoking is not permitted. The presence of factors that influence interpretation of the result of the test must be recorded. (e.g. medication, inactivity, infection)

    • After collection of fasting blood sample, the subject is required to drink 75g of anhydrous glucose in 250-300ml water within 10 minutes

    • Another blood sample must be collected 2 hrs after finishing glucose drink

    It is confirmed if:

    • Fasting blood sugar >7.0mmol/L or

    • 2 hours Post-sugar - >11.1 mmol/L

    1-2% of women will develop gestational diabetes and they are having risk to develop diabetes mellitus type 2 in future.

    What are the risk factors?

    • Women >35 years old
    • Maternal obesity (BMI >30)
    • Family background of diabetes mellitus among 1st degree relatives
    • Previous history of big baby (>4 kg)
    • History of gestational diabetes in previous pregnancy
    • History of unexplained fetal death
    • History of congenital anomaly in previous pregnancy

    What are the symptoms?

    • Usually no symptom

    • Sometime may develop:
    – Excessive passage of urine
    – Extreme thirsty
    – Excessive passage of urine at night or during sleep
    – Extreme hunger
    – Infection
    – Sweet smelling breath
    – Irritability
    – Unusual weight loss
    – Nausea
    – Vomiting

    What are the effects to the mother and the baby?

    Effects to the pregnant mother

    1. Pregnancy induced hypertension (2 times greater risk to develop hypertension compared to non diabetic pregnant mother and it is because of the effect of estrogen hormone and the abnormality of blood vessel)

    2. Excessive amniotic fluid + premature rupture of amniotic membrane

    3. Premature labour give rise to premature baby

    4. Infection

    Urinary tract infection (d/t high sugar level in the urine, therefore bacteria are more likely to grow)

    Candidiasis of the vulva and vagina

    Effects to the fetus/baby

    1. Early pregnancy

    Spontaneous abortion

    Congenital anomalies (heart, limbs, spinal cord)

    2. Later pregnancy

    Big baby (more than 4kg at birth, it is because when the mother’s blood sugar levels are elevated, the sugar crosses the placenta and cause blood sugar level in fetus increased as well. The fetus in such a setting is “overfed” and grows large, with increased deposits of fat and glycogen. Large baby has a significantly increased risk for birth trauma. Besides that, high blood sugar level in fetus also causing overproduction of insulin in the fetus. Insulin promotes growth, therefore the fetus grow rapidly)

    Fetal death (Due to poor oxygen delivery to the fetus)

    Polycythemia (A condition marked by an abnormally large number of red blood cells in the circulatory system as a compensation of lack of oxygen.)+ hyperbilirubinemia (excessive bilirubin in blood due to excessive breakdown of the red blood cell and it cause jaundice)

    3. After delivery

    Low blood sugar level (Fetus exposed to high blood sugar level results in over production of insulin hormone. When it is delivered and the sugar supply is cut off but the insulin secretion persists, the infant can develop extremely low sugar level, especially during the early hours of life. This can be serious since the brain utilizes sugar as a major substrate. Prolonged low blood sugar level can produce brain dysfunction.)

    Jaundice (yellowish discoloration of the whites of the eyes, skin, and mucous membranes)

    Breathing difficulty (because of delayed fetal lung maturation and the harming effect of insulin hormone on lung maturation)

    What should I do to prevent/ overcome this problem?

    Principles:

    · Control maternal blood sugar levels

    · Early delivery (at least not post date) in an appropriate site based on fetal heart testing and lung maturity

    Oh dear. My MGTT test result are out and because the phone number use belong to Mr. hubby, the nurse just called him today. He message me to let me know that I need to do PSP BSP test.

    First I google for PSP and get "Pemeriksaan sendiri payudara" as the result. Weird, so I text him and ask what is PSP? He explain that it's another glucose test.. and I have DIABETES :-O!!!

    Bad news..bad news.. so I google and found out that he actually refer to BSP. Its a test to see whether I have gestational diabetes. The test would required my blood to be tested 4x (8am, 11am,5pm and 9pm). I need to do the test because my glucose level in blood is higher than the normal <7 in my last MGTT test.

    I really need to control my diet now, because having diabetes is not only dangerous for me, but for our sweet junior. So sad... at least I get to know it early and do what it take to control it from becoming worst.
